Community care

Council delivers multiple services under our extensive community care program with assistance from our amazing group of Domestic Care Workers.

One program is the Victorian Home and Community Care Program for Younger People (VIC HACC PYP) which is available for those aged under 65 years. Another is the Commonwealth Home Support Program (CHSP) which is available for people aged 65 years and over. Council also delivers brokerage services for multiple case management agency, Veterans Affairs and TAC.

Victorian Home and Community Care Program for Younger People

This program supports Victorians from birth to 65 years, and Aboriginal and Torres Strait Islander people from birth to 50 years, if their capacity for independent living is at risk. You can apply for this program directly with council, no third-party referrals are required.

Commonwealth Home Support Program

This program helps older Australians access entry-level support services to live independently and safely at home for as long as possible. This program is assessed via an application to My Aged Care who then refer your application to council once it has been approved.

Meals-on-Wheels

Do you or someone you know need meal preparation assistance? Our Community Care team can help assess if they qualify for our meals-on-wheels service which is available Monday-Friday. You can apply for this program directly with council, no third-party referrals are required.

Public holiday support

On public holidays, our team can only provide essential services which includes personal care. Meals-on-wheels is delivered on the business day prior, shopping support services are rescheduled for a different time that week, and domestic services are cancelled until the following fortnight.
